Frequently Asked Questions (FAQ) for families

  • What is CHDP? +

    The Child Health & Disability Prevention (CHDP) Program provides complete health assessments for the early detection and prevention of disease and disability in children and youth. It is a program offered to eligible children and youth with no-cost. Children may receive well-child care visits, according to CHDP Periodicity Schedule, from birth through 20 years of age if they have Medi-Cal and from birth through 18 years of age if they are low-income with no Medi-Cal.
  • What is a CHDP health exam? +

    The CHDP examination is a complete head to toe health evaluation of a child or youth covering health history, physical exam, review of dental, nutritional and developmental status, screenings of hearing and vision, tests for anemia and lead exposure, immunization, and health education. Occasionally additional tests may be included.
  • Are CHDP examinations the same as the regular check-up a child receives? +

    Yes, but the CHDP exam requires the provider (medical doctor) to include all the tests mentioned in the previous question.
  • Does the child receive the same service at each CHDP health checkup? +

    No. The services a child receives are based on the child's age and health status according to the AAP Bright Futures periodicity schedule. The periodicity schedule lists the frequency of exams, the type of tests and immunizations and the interval between CHDP exams that are required for each age. Every CHDP provider is required to do the check-ups according to this schedule.
  • How often can my child get a CHDP physical? +

    It depends upon how old the child is. If the child does not have Medi-Cal managed care nor has regular Medi-Cal, he/she is eligible for physical exams according to the Bright Futures periodicity schedule.
  • What if a problem is found during the CHDP exam? +

    If a provider finds a problem in any part of the exam, the child and the family may receive advice, a specific treatment (such as a prescription) or a referral for additional examinations, diagnosis and treatment.
  • What is a CHDP referral? +

    One of the goals of the CHDP Program is to find any medical, dental, nutritional and developmental problems that a child may have before the problems become too severe for treatment. Once a medical, dental, nutritional or developmental problem is identified during CHDP health exam, the child may need further diagnosis and/or treatment of that problem. The CHDP provider is obligated to make the referrals to assist the family to obtain the care their children need.
  • What kinds of resources are available to pay for a referral? +

    If your child has Medi-Cal, it may pay for the referrals. If your child does not have Medi-Cal, it may be paid by CHDTP, which is a treatment program and operates through University Medical Center (UMC). California Children Services (CCS) also provides help to children whose medical conditions meets CCS requirements. There also may be other pay sources that the child would be eligible for. CHDP nurses can help to explore all possible resources.
  • Who pays for the specialty care? +

    Children who have full scope Medi-Cal can use their insurance to pay for the specialized care. For families without insurance, many resources depend on the type of problem and family situation. The family can contact the CHDP office and our staff will be happy to assist the family to obtain resources.
  • Does the CHDP Administrative Office help with the referral process? +

    The CHDP staff is available to assist families and providers find and obtain needed services.
  • Can I go through the Gateway process if I do not have a CHDP exam? +

    No. To go through the Gateway process the child or youth must be scheduled to undergo a CHDP well-child health assessment. Once the child or youth is at the CHDP provider's office, the staff will begin the Gateway process to determine what insurance the child or youth is eligible to receive. If no insurance is found, the family is provided with two documents:- Pre-visit brochure- CHDP Pre-enrollment application (DHS 4073)
  • Do I need to show proof of residency or income to go through the Gateway process? +

    No! You do not have to provide proof of family income or residency.
  • What are the age requirements for Gateway? +

    Children and youth up to age 19. Family income must be at or below 266% of Federal Income Guidelines and a California resident.
  • Will Gateway affect my immigration status? +

    No! Any child can retain CHDP Gateway services based on age, income, and residency.
  • How does the Gateway process work? +

    The Gateway process provides the means for children and youth up to age 19 who are pre-enrolled in Medi-Cal at the time of a CHDP health assessment to receive temporary full scope, no-cost Medi-Cal benefits, and start the application process for Medi-Cal. If children do not qualify for Medi-Cal, CHDP Gateway ensures well child visits.
  • What is CHDP Gateway? +

    CHDP Gateway is an automated pre-enrollment process for children and youth who are uninsured and low to moderate income.
  • I do not speak English, do the clinics have an interpreter? +

    Yes, our CHDP Providers have interpreters available or access to one for non-English speaking clients.
  • I do not have an eye doctor. Could you give me some names of eye doctors who accept Medi-Cal? +

  • I don’t know any dentists that would take young children with Medi-Cal. Where can I find one? +

  • Where can I go when my child needs to see a dentist, but only has emergency Medi-Cal? +

  • How does the Gateway process work? +

  • Can I get a WIC physical for my child? +

    Yes, the CHDP exam document is acceptable for enrollment in WIC programs. If you have just had a CHDP physical for your child, you can contact your doctor and request the WIC paperwork to be completed.
  • What does a family need to bring to a CHDP visit to confirm eligibility? +

    Families with Medi-Cal must bring their Medi-Cal cards; families that have no insurance must complete and sign a form declaring their income at the time of the visit. Families of children attending a Head Start or State Preschool program must obtain a CHDP form from the school and bring it to the CHDP visit.
  • Does my family qualify for CHDP? +

    That is determined on the basis of several factors such as your family income, number of people living in your household, your child's age, etc.
  • Who is eligible? +

    All children who have Medi-Cal ages 0-20 years, low-income children with no health insurance ages 0-18 years who meet CHDP eligibility, foster children, children who attend Head Start and State Preschool ages 3-5 years.
