Dr. Blaze graduated from University of California, Irvine, School of Medicine. She completed her Residency in Family Medicine and Fellowship in Obstetrics at Ventura County Medical Center. Dr. Blaze is Board Certified by the American Board of Family Medicine; she is a member of Alpha Omega Alpha Honor Society and the American Academy of Family Physicians.
