Goals
Assist adults, 50 years and older, who have a chronic medical condition, to have the best possible health outcomes.
Services
IHSS/APS (In-Home Support Services/Adult Protective Services). Public Health Nurses conduct home visits to In-Home Support Services recipients to assess medical case management needs. We also assess health needs of clients referred to Adult Protective Services.
Senior Case Management serves frail seniors who are functionally impaired or at risk for being institutionalized. We try to help elderly persons remain in their own homes by linking them to community resources. We provide home visits, assessments, education and referral for health and human services needs.
Preventive Health Care for Adults (PHCA): Non-frail older Ventura County adults (50+) , including low income and ethnic minority adults can get help from Public Health Nurses. We provide outreach, community health education and activities to increase access to health services and comprehensive health assessments.
