Morbidity Report Form for Medical Providers

Fill out and FAX the Confidential Morbidity Report Form to report a suspected communicable disease case or outbreak. This form lists reportable diseases and reporting time mandates.

Meaningful Use Data: Notice to Professionals

We are unable to receive EHR for immunization registry, laboratory reporting, and syndromic surveillance pending further specification from the Centers for Medicare and Medicaid.
