Transitions


Program Description:


This program serves transitional age youth (TAY), ages 18 to 25 years old, with serious mental illness, by addressing their full spectrum of needs.  Transitions focuses on a client driven model of care, which integrates wellness and recovery into the array of services, including psychiatric treatment, individual therapy, intensive case management services, group treatment and rehabilitation services.  Services and supports are provided both in the clinic, as well as field-based services within homes, community, and the TAY Wellness and Recovery Center.  Peer staff, or “Recovery Coaches,” support consumers in the achievement of their wellness and recovery goals.  The program serves the Oxnard, Ventura, Conejo and Simi Valley regions of Ventura County and has been effective in expanding access to services to traditionally un-served and underserved TAY in these areas.

Successful Outcomes:


Each year this program continues to grow as services are tailored to the unique developmental needs of this age group.  During this last year, over 580 clients were served county wide. 11 young adults were assisted with finding appropriate independent housing within the community.  11 Transitions clients were supported through their goal of developing and beginning their higher education program aspirations.  36 clients were assisted in finding employment. 


Services and Supports:


Evidence Based Practices are utilized to provide group therapy, rehabilitation services, social skills, life skills, and developing healthy relationships and classes on, Mood Management.  Taking a holistic approach to healing classes in creative expression, Lyrical Soul classes, drumming circles, art therapy,  Mind & Body, health and fitness, equine therapy groups and parenting classes are offered.  Evidence based practices, including:  Seeking Safety and DTQI Individual therapy; medication management; rehabilitation services, case management services.


Rehabilitative/Community Activities/Events

  

Dia de Los Muertos, TAY Picnic, HoliTAY, Parenting Group, Rewards Incentive Client Group, Voices 2013, Hollywood, CA, The Painted Brain Workshop, LA, CA all serve to engage clients in safe meaningful experiences with peers and in their community.
