Application for 2015-2016 Certificate of Fetal Death

Authorized Persons: Certified copies of the public portion of fetal death certificates are not restricted and may be issued to any person requesting a copy upon payment of the required fee. The confidential portion of the fetal death certificate may be issued only to authorized persons.

In Person

Visit the Vital Records Office at 2240 E. Gonzales Road, Suite 150, Oxnard, between 9am to 4pm Monday through Friday to complete an application form. You must present a valid, government issued photo ID at the time of purchase only if you are paying by personal check or credit card or requesting the confidential portion of the certificate. If your application is approved, your request will be processed while you wait. If no record is found, the fee is retained to cover the cost of the file search.

By Mail

Mail a completed, fetal death certificate application form to the Vital Records office. Our address is at the top of the application form. Mail orders paid by personal check are held for 14 working days. Mail orders submitted with a cashier’s check or money order are processed the day they are received if the certificate has been registered. If no record is found, the fee is retained to cover the cost of the file search.

Fee: $18.00 per certified copy.
